Everything an egg donor recipient needs to know about egg donation from the IVF egg donation expert – Dr Randy Morris

Egg donation can be considered in several different situations. The most obvious is when a women who wants to become pregnant does not have ovaries. This usually occurs because she has had surgery to remove her ovaries but there are also situations in which a woman could be born without ovaries.

Women who have ovaries may also, at times, need an egg donor. Common situation include

— Advanced age
— Poor ovarian reserve
— IVF problems
— Genetic problems

The first step in the process is to select an egg donor. An ideal egg donor is young, healthy and does not have a history of infertility herself. Donors can be someone you know such as a relative or friend or they can be anonymous. Once you have selected a possible donor applicant, she must go through extensive testing to look at her ovarian function and make sure she does not have any potential infectious diseases.

For you, it is important to have a good evaluation of your uterine cavity. This can be accomplished with a hysteroscopy or a saline ultrasound.

The actual egg donation treatment can be completed in a number of ways. The donor will take fertility medications to stimulate the maturation of multiple eggs in her ovaries at the same time. After several days, the eggs will be retrieved and fertilized with the sperm of your partner or a sperm donor. The embryos are left to develop in the laboratory for several days.

At this point, an embryo can be transferred into your uterus provided your uterus has been prepared with medication to receive an embryo at that moment. This requires precise synchronization of your cycles with the donor cycle

Another option is to freeze the embryos so they can be placed into you uterus at any convenient time. This way, you don’t have to worry about synchronization.

It is also possible to freeze the donated eggs before fertilization. There are a few egg banks where you can get frozen donor eggs but this is a pretty expensive option.

Pregnancy success rates with egg donation are very high provided that the donor you selected is a young donor. We generally recommend donors under the age of 25.

The risk for multiple pregnancy is higher with donor eggs so you should never transfer more than one embryo at a time to you uterus. Donors generally produce a lot of eggs so there is a good chance for having extra embryos that can be frozen for your own future use.

Pregnancies conceived with donor eggs may have a higher risk for blood pressure problems. Be sure you discuss that with your doctor.

Cushing syndrome is a group of conditions caused by high levels of circulating cortisol or related corticosteroids.
A gout attack occurs suddenly, often at night. The affected joint is swollen, red and warm to touch. The initial attack usually involves only one joint, most often in the big toe, and lasts a few days. Subsequent flares may involve multiple joints and can last for weeks, with shorter intervals of remission in between.
Gout is caused by deposits of urate crystals in the joints, which may happen when blood levels of uric acid are too high. Uric acid is a waste product from the breakdown of purines, a major component of DNA and RNA. Because body’s cells are constantly renewed, uric acid is constantly produced and removed in urine. High serum urate level may result from decreased urate excretion, increased urate production, and/or overconsumption of purines.
The most common cause is the kidney’s decreased ability to excrete uric acid, which can be hereditary, or a result of kidney diseases, medications such as diuretics, alcohol use, or lead poisoning. Increased production of urate may occur in conditions with abnormally high cellular turnover or cell death, or in obesity, as urate production increases with greater body surface area. Overconsumption of purine-rich foods can contribute to higher urate level, but is rarely the only cause.
High concentration of urate promotes its precipitation and formation of crystals in joints, causing inflammation and pain. Because uric acid crystals form more easily in cooler temperatures, gout more often affects joints of the extremities. Urate crystals may also deposit under the skin around the joints, forming hard lumps, called tophi, that are not usually painful but can limit joint motions and lead to deformities. Joint damage caused by gout also increases the risk of developing osteoarthritis. In the kidneys, urate deposits may form stones, blocking urine flow.
Gout is more common in men. It often starts in middle age in men, and after menopause in women. Other risk factors include diets, obesity, other health conditions, family history, and surgery or trauma.
Diagnosis requires detection of urate crystals in synovial joint fluid. Because not all people with high serum urate levels develop gout, a blood test alone is not sufficient for diagnosis.
Acute gout attacks are treated with anti-inflammatory drugs, such as NSAIDs, regular use of which may also help prevent or reduce frequency of attacks. Because tophi can be dissolved by lowering serum urate, medications that block urate production or increase urate excretion may be used for this purpose.

Gout may be triggered by an increasing level of uric acid in our body. It guarantees an intense pain usually at the big toe joints and is more likely to flare at night (approximately 2.4 times more frequent than the daytime). This is because the concentration of uric acid increases while we’re sleeping. While asleep, through breathing and sweating, our body loses moisture. Less water content in the blood equals higher concentration of uric acid and may lead to hyperuricemia, the precursor to gout.

Holding our breath while sleeping is out of the option so the other factor that we can control to manage our uric acid level is through diet. Avoid eating a diet rich in red meat and seafood with high purine content. It’s also highly risky to consume alcohol and beverages sweetened with fruit sugar. Further treatment includes several types of drugs that can block the uric acid production such as Allopurinol and Febuxostat.

Painful and embarrassing, no one wants to talk about hemorrhoids. The simple truth is once these swollen veins become a problem people can’t sit back and ignore it.

“It’s because it’s thrombosed, in other words that’s a fancy word for it, it has a small clot. It distends the skin and it becomes real painful,” says Lee Memorial Health System general surgeon Dr. Steve Moenning.

Now there’s a new, less-invasive treatment that makes these bleeding blood vessels disappear. Where surgery used to be the only option, the FDA approved a bipolar device that destroys the troublemaker.

“We still have to put the patient to sleep, but bipolar is a fancy way of saying energy goes from one part of the device to another,” says Dr. Moenning.

This procedure required is one session and patients go home the same day. Compared to traditional surgery, which cuts out the offending clot leaving patients with an uncomfortable healing process. The bipolar method is cutting-edge treatment with no cutting.

“It shrinks the blood flow into the hemorrhoid, which then shrinks the internal hemorrhoids. It doesn’t extend deeper to involve the muscle tissue. So there’s no surgery in terms of cutting out the tissue. There’s no sewing up,” says Dr. Moenning.

The procedure is quick and relief is almost immediate.

“It’s phenomenal. We can get rid of the internal hemorrhoids and frequently shrink the external hemorrhoids in a painless fashion,” says Dr. Moenning.

More serious cases may still require surgery, but this technique is sapping the strength out of most hemorrhoids.
